29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about November, 19, 2014 an employee used an extension ladder to gain access to a roof where the ladder's length did not extend three feet past the roof's edge, exposing the employee to a fall of 20'. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. On or about November, 19, 2014 an employee was not provided training to recognize or minimize the hazards associated with using an extension ladder, exposing the employee to the hazards associated with ladder use. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
