29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about November 20, 2014 at worksite 10521 Hillcrest Rd, Kansas City Missouri.-employees were exposed to serious fall hazards in that they had not been trained in fall hazard recognition and the proper installation or use of the equipment, procedures and systems designed to mitigate those hazards. 29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder. (a) On or about November 20, 2014 at worksite 10521 Hillcrest Rd, Kansas City Missouri.-employees were accessing a working surface on a low pitch roof by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed. Abatement certification for this violation is not required for this item.
