29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about November 17, 2014, the employer did not provide eye protection to employees using nail guns to perform framing activities. The employees were exposed to the hazard of an eye injury.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems.  On or about November 17, 2014,  employees who were performing framing activities on the upper frame of a newly constructed residential home were not provided fall protection exposing the employees to an unprotected fall hazard of approximately 11 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about November 17, 2014, the employer did not ensure employees engaged in framing activities were trained to recognize and avoid fall hazards and the procedures to be followed in order to minimize or avoid those hazards.
