29 CFR 1910.134(l)(1): The employer did not conduct evaluations of the workplace as necessary to ensure that the provisions of the current written program were being effectively implemented and that it continues to be effective: (a) Concrete Pipe & Precast, LLC - The employer did not evaluate the written respiratory protection program to ensure that when air sampling results indicated an overexposure to silica on May 13, 2014, the employee(s) were wearing the appropriate respirators while working in the production area, on or about November 21, 2014.
