29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided prevented pull from being directly transmitted to joints or terminal screws: a) U.S.S. Eisenhower, Flight deck, Catapult System #1: On or about 11/21/2014 and times prior thereto, employees were operating a portable electric "Air Hog" blower fan, Model# WM121LOW1 with broken and missing strain relief. Upon inspection of the cord, the fan would de-energize and turn off, when the cord was moved again it would spontaneously re-energize power and turn back on with movement of the cord, exposing employees to electrical shock and electrical burns. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.134(e): Safety guards were not mounted as to maintain proper alignment with the wheel, and the guard and its fastenings shall be of sufficient strength to retain fragments of the wheel in case of accidental breakage: a) U.S.S. Eisenhower, Flight deck, Catapult System #1, Space #12: On or about 11/21/2014, a portable pneumatic right-angled grinder was missing its protective guard exposing employees to fragmentation injury, lacerations or death. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Eisenhower, Flight deck, Catapult System #1, Space #12: On or about 11/21/2014 and times prior thereto, employees were performing operations with a temporary electric extension cord with numerous cuts and cracks in the outer-jacket of the cord. The cords plug was pulled away from the cord exposing the internal insulated wires, exposing employees to electrical shock or electrical burns. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
