29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided prevented pull from being directly transmitted to joints or terminal screws:    a) U.S.S. Eisenhower, Flight deck, Catapult System #1: On or about 11/21/2014 and times prior thereto, employees were operating a portable electric "Air Hog" blower fan, Model# WM121LOW1 with broken and missing strain relief.  Upon inspection of the cord, the fan would de-energize and turn off, when the cord was moved again it would spontaneously re-energize power and turn back on with movement of the cord, exposing employees to electrical shock and electrical burns.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
