Core Link Area - On or about and times prior to November 19, 2014, the employer did not assess the need for personal protective equipment for employees working on top of paper cores that were unloaded from an open top trailer, thus exposing employees to a fall hazard of approximately 12.5 feet onto concrete flooring.
