29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 ppm:      a) On or about November 22, 2014, an employee working in the production area was overexposed to carbon monoxide at an 8 hour time weighted average of 213.50 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 427%.    b) On or about November 22, 2014, an employee working in the production area was overexposed to carbon monoxide at an 8 hour time weighted average of 183.50 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 367%.    c.) On or about November 22, 2014, an employee working in the production area was overexposed to carbon monoxide at an 8 hour time weighted average of 201.6 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 403%.      d.) On or about November 22, 2014, an employee working in the production area was overexposed to carbon monoxide at an 8 hour time weighted average of 251.7 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 503%.    e.) On or about December 10, 2014, an employee working in the production area, operating a forklift, was overexposed to carbon monoxide at an 8 hour time weighted average of 97.0 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 194%.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):    a.) On or about November 22, 2014, the employer failed to implement adequate engineering and/or administrative controls to control employees exposure to carbon monoxide while working in the production area. The production worker was overexposed to carbon monoxide at an 8 hour time weighted average of 213.50 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 427%.   b.) On or about November 22, 2014, the employer failed to implement adequate engineering and/or administrative controls to control employees exposure to carbon monoxide while working in the production area. The production worker was overexposed to carbon monoxide at an 8 hour time weighted average of 251.7 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 503%.     e.) On or about December 10, 2014, the employer failed to implement adequate engineering and/or administrative controls to control employees exposure to carbon monoxide while working in the production area, operating a forklift.  The forklift operator was overexposed to carbon monoxide at an 8 hour time weighted average of 97.0 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by 194%.

29 CFR 1910.1200(h)(1):      Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:           1.) On or about November 24, 2014 the employer failed to train employees on the elements of a written hazard communication program including the hazards of the chemicals in their work area.  Employees were exposed to hazards from carbon monoxide gas from the exhaust of a propane powered forklift used in the facility.
