29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Siding America, Inc. at Conifer Street and Redwood Street, Fort Collins, CO 80524:  On and before November 7th, 2014, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  The condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  (a)Siding America, Inc. at Conifer Street and Redwood Street, Fort Collins, CO 80524:  On and before November 7th, 2014, the employer allowed employees to use an orange extension cord that was not grounded with a Rigid circular saw, model R3205.  The condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a)Siding America, Inc. at Conifer Street and Redwood Street, Fort Collins, CO 80524:  On and before November 7th, 2014, the employer allowed employees to use an orange extension cord with frayed insulation with a Rigid circular saw, model R3205, to cut siding.  The condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  (a)Siding America, Inc. at Conifer Street and Redwood Street, Fort Collins, CO 80524:  On and before November 7th, 2014, the employer did not ensure that the pump jack scaffold used in siding was erected under the direction of a competent person, because it included the following deficiencies:  1)The scaffold failed to include mid rails;  2)The scaffold failed to included end rails;  and  3)The materials platform, used as a top rail, was not secured, in that employees could lean against it and inadvertently push it of the horizontal support, creating a fall hazard.   The condition exposed employees to a fall hazard of 7 feet 9 inches.
