On or about 11/24/14 located at 210 Ashland CT., Alpharetta, GA: Employees were framing the walls on the 2nd floor of a house that was under construction while working at the edges of the staircase floor hole opening, the employer did not ensure that guardrails or floor hole coverings was used by the employees while exposed to fall hazards of approximately up to 11.7 feet.
