29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about 11/24/14 located at 210 Ashland CT., Alpharetta, GA: Employees were framing the walls on a house and were observed swinging the lumber over the heads of the other employees, which were without hard hats exposing them to a struck by hazards.

On or about 11/24/14 located at 210 Ashland CT., Alpharetta, GA:  Employees were observed installing 2 x 4 lumber on to a house under construction with a pneumatic nail gun  and without safety glasses.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.   On or about 11/24/14 located at 210 Ashland CT., Alpharetta, GA: Employees were framing the walls on the 2nd floor of a house that was under construction while working at the edges of the staircase floor hole opening, the employer did not ensure that guardrails or floor hole coverings was used by the employees while exposed to fall hazards of approximately up to 11.7 feet.
