29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a)  On or about November 25, 2014, the employer did not assure that the ingoing nip points and cutting knives associated with the 400 line were properly guarded.  The employees operating the machine were exposed to amputation hazards due to the door interlocks being bypassed.   b)  On or about November 25, 2014, the employer did not assure that the bag sealers at the end of the 75-3 line were guarded.  Employees were exposed to burning injuries when loading bags of product into the sealers.   c)  On or about November 25, 2015, the employer did not assure that the bagger area associated with M19 was properly guarded.  An employee was able to reach under a guard to remove a bag and suffered a severe hand laceration.   d)  On or about January 15, 2015, the employer did not assure that the ingoing nip points and cutting knives associated with the acquisition section of M19 were properly guarded.  The employees operating the M19 line were exposed to amputation hazards due to an access door being open during production.
