29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about November 24, 2014, and times prior thereto, inside of the apartment complex, employees engaged in carpentry activities while working below other workers were not wearing hard hats and exposed to head injuries from falling tools and materials.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe. On or about November 24, 2014, and times prior thereto, inside the building, employees used a Chicago 12 inch miter saw to cut material with the blade guard wedged in the up position exposing them to amputation hazards.
