29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable. a) In the pipe welding and painting building, on or about November 21, 2014, and at times prior to, the floor was flooded and not maintained in a dry condition when it rains, exposing employees to slip and fall hazards.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a) On or about November 21, 2014, and at times prior to, employees operated the Big Red forklift that did not have a functioning back-up alarm or a functioning horn. Employees used the forklift to move big pipes and materials in the yard, exposing employees to struck-by and crush hazards.
