29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On or about November 13, 2014 at worksite 1702 NE Chapel Woods Drive in Lees Summit, Missouri- Employee was using a pnematic nail gun to secure shingles to the roof without having any means of eye or face protective equipment.   Abatement certification for this item is not required.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems or personal fall arrest systems:  On or about November 13, 2014 at worksite 1702 NE Chapel Woods Drive in Lees Summit, Missouri- Employees were performing roofing operations at an elevated height on a steep pitch roof without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.   Abatement certification for this item is not required.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about November 13, 2014 at a job site located at 1702 NE Chapel Woods Drive in Lees Summit, MO, workers were exposed to serious fall hazards in that they had not been trained in fall hazard recognition and the proper installation or use of the equipment, procedures and systems designed to mitigate those hazards.   Abatement certification for this item is required.
