Paint booth: Employees required to use the 3M Versaflo M-100 Powered Air Purifying Respirators equipped with TR 3710N High Efficiency Particulate (HE) filter for painting operations were not provided with a medical evaluation to determine the employees ability to use it. Condition noted on or about 11/21/2014.
