29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At this construction site, a portable ladder used by employees to access an outdoor advertising sign's mechanical and electrical control room did not extend 3 feet above the landing. Employees were exposed to falls greater than 6 feet.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    At this construction site an employee was using an "A" frame ladder in the closed position. Use of this ladder in this manner contributes to instability exposing this employee to falls.
