29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about November 25, 2014 at the above addressed job site, employer did not establish or implement an energy control program describing energy control procedures, employee training and periodic equipment inspection for employees performing maintenance or servicing of equipment, such as but not limited to Amada HFE 220-4S press break, AccurPress 71008 press break, Di-Acro press break and Strippit hole puncher; exposing employees to caught by and caught in hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. On or about November 25, 2014 at the above addressed job site, employees were exposed to caught by hazards while working with a press brakes such as Amada HFE 220-4S, AccurPress 71008 and Di-Acro press brake without point of operation guarding and/or alternate measures that provide effective protection.    b. On or about November 25, 2014 at the above addressed job site, employees were exposed to caught by hazards while working with Strippit hole puncher that lacked a guard on its point of operation.    c. On or about November 25, 2014 at the above addressed job site, employees were exposed to caught by hazards while working with Jet vertical band saw that lacked guarding for the unused portion of the cutting blade.
