29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace.    a) On or about November 20, 2014, in the welding bay of the shop located at 4855 Chaincraft Rd., Cleveland, Ohio 44125, an employee performing pneumatic spray painting, of a extension table on the Shannon press, was provided a half mask air purifying respirator to use and was not first medically cleared by the employer to use the respirator.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    a) On or about, November 21, 2014, while operating the Lucas 542B horizontal boring machine located at 4855 Chaincraft Rd., Cleveland, Ohio 44125, the machinist was exposed to the unguarded rotating shaft and cutting head of the boring machine while operating it.    b) On and before, November 21, 2014, while operating the Rockland lathe located at 4855 Chaincraft Rd., Cleveland, Ohio 44125, the machinist was exposed to the unguarded rotating chuck while performing milling operations.
