29 CFR 1926.416(a)(1): An employee was permitted to work in proximity to an electric power circuit that the employee could contact in the course of work and was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means:     (a) On or about 10/18/14, On or about October 18, 2014, an employee received severe electrical burns while working on the Electrical Switch Gear Tie Breaker unit of SS41B.  An energized bus bar operated at approximately 480 volts and was not protected from accidental contact.

29 CFR 1926.416(a)(3): Before work began the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring a person, tool, or machine into physical or electrical contact with the energized electric power circuit:     (a) Electrical Switch Gear SS41A & SS41B - On or about 10/18/14, the employer did not ascertain by adequate inquiry or direct observation, or by instruments of the specific parts of the energized Electrical Switch Gear Tie Breaker SS41A and SS41B electrical power circuits which were not safely located or de-energized.  Additionally, the employer did not adequately advise all employees of the locations of hazardous energized lines and of the necessary measures required to be taken to ensure that exposure to electrical hazards was eliminated.
