29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination: a) 1491 Zeager Road, Elizabethtown PA; Wash Bay - On November 25, 2014 and at times prior thereto, employees entered tank trailers to remove chocolate and the employer did not document through written certification, that all hazards in the permit space were eliminated. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
