Observed on or about 11/25/14.

Employees wore North 7700 half-mask air purifying respirators with P100 cartridges when they worked in the following areas:  Large TV; Glass Room (Shred Room) including forklift drivers, and the last position on the Roll-off Line, and the Consumer Lines. Lead dust concentrations ranged from 90 to 405 micrograms in a 100 square centimeter area.   Observed on or about 11/25/14.

Wipe samples of the floor in the warehouse contained lead.  Observed on or about 11/25/14.

29 CFR 1910.1025(l)(1)(i): Employee(s) working in an area where there is potential exposure to airborne lead at any level were not informed of the content of Appendices A and B of 29 CFR 1910.1025:    a) Universal Waste Solutions, LLC (Warehouse) - Employees were exposed to airborne lead and the employer did not inform them of the content of Appendices A and B of the OSHA Lead Standard, 29 CFR 1910.1025.  Employee exposure monitoring was conducted on 1/15/15 showed that employees working in the Production Lines, Roll-off Line, Large TV Breakdown, Logistics, Baler Area, Glass Room and forklift drivers were exposed to airborne lead.  Observed on or about 1/15/15.
