29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    On or about November 26, 2014, at the above addressed job site, an employee operating a rough terrain forklift with a personnel carrier attachment did not complete training and evaluation on the operation of the forklift, exposing employees to the hazards associated with improper operation of the forklift and fall hazards.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.150(c)(1)(iv): One or more fire extinguishers, rated not less than 2A, were not provided on each floor:    On or about November 26, 2014, at the above addressed worksite, employees engaged in residential carpentry operations near an open-flame space heater did not have access to a fire extinguisher in the basement of the building, exposing employees to fire hazards.    There is no abatement certification or documentation required for this item.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    On or about November 26, 2014, at the above addressed jobsite, a scaffold was moved horizontally while an employee was working from the scaffold, exposing the employee to a fall hazard of approximately eight (8) feet.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
