29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems: a) On November 26, 2014, at the jobsite located at 318 Palmer Street in Bowling Green OH: the employer did not assure employees were protected from fall hazards while installing roofing material on a commercial building. The employees were exposed to an 18 foot fall to the ground below. Ludwig Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.501(b)(11), which was contained in OSHA inspection number 449613, citation number 1, item number 1a and was affirmed as a final order on June 25, 2012 with respect to a workplace located at 3700 Speedway Drive in Findlay, Ohio.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) On November 26, 2014, at the jobsite located at 318 Palmer Street in Bowling Green, OH: the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems. Employees were exposed to falls of approximately 18 feet to the ground below. Ludwig Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.503(a)(1), which was contained in OSHA inspection number 449613, citation number 1, item number 1b and was affirmed as a final order on June 25, 2012 with respect to a workplace located at 3700 Speedway Drive in Findlay, Ohio.
