29 CFR 1910.266(i)(7)(i): The employer did not assure that each employee, including supervisors, had received first-aid and CPR training meeting at least the requirements specified in Appendix B:    On or about November 25, 2014 and times prior thereto, employees of GM Lawrence and Company DBA Lee Wilson & Co. Gin performing  cotton gin operations at the facility located at or near 5765 S. Highway 61 in Wilson, Arkansas.  The employer did not assure any  employees, including supervisors, were trained in first-aid and CPR in the event of emergency.

29 CFR 1928.57(a)(6): 29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually  thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved:    On or about November 25, 2014 and times prior thereto, employees of GM Lawrence and Company DBA Lee Wilson & Company Gin performed cotton gin operations at the facility located at or near 5765 S. Highway 61 in Wilson, Arkansas.  The employer failed to instruct employee in safe operation, adjustment and servicing of the Lummus Dor-Les bale press.  This condition exposed the employee to caught in and struck by hazards.

29 CFR 1928.57(a)(6)(iii): The employer did not stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment, except where the machine must be running to be properly serviced or maintained, in which case the employer shall instruct employees as to all steps and procedures which are necessary to safely service or maintain the equipment;    On or about November 25, 2014 and times prior thereto, employees of GM Lawrence and Company DBA Lee Wilson & Co. Gin performed cotton gin operations at the facility located at or near 5765 S. Highway 61 in Wilson, Arkansas.  Adjustment was performed on an energized Lummus Dor-Les bale press with no effort or requirements to stop engine and disconnect the power source, exposing employees to caught-in hazards.

Gin performed cotton gin operations at 5765 S. Highway 61 in Wilson, Arkansas.  Employees operated a Lummus Dor-Les cotton bale press which was not guarded to prevent employees from coming into contact with the point of operation.
