29 CFR 1910.101(b): "Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. On or about November 12, 2014, at the center of the warehouse , the employer allowed employees to work and use compressed gas cylinders without being properly chained or secured. a: Propylene Flammable compressed gas chain was not secured. b: Propylene Flammable compressed gas cylinder was sitting in the middle of the warehouse without being secured. c: Helium Compressed gas cylinder was sitting in the middle of the warehouse without being secured.

29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. On or about November 21, 2014, the employer allowed employees to operate a Supermax Lathe machine with a control knob that was broken and disengaged from machine exposing live electrical wires and machine parts.
