29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   (a) On or about November 20, 2014 at worksite 10526 Grandview Rd, Kansas City, Missouri.-employees were exposed to a fall hazard by working at heights of approximately 14- 16 feet without any form of fall protection.

29 CFR 1926.1053(b)(1):    Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   (a) On or about November 20, 2014 at worksite 10526 Grandview Rd, Kansas City, Missouri.-employees were exposed to a fall hazard by using a ladder which extended approximately 2 feet above the landing surface of the roof to which he gained access.    Abatement certification for this item is not required.

29 CFR 1926.1053(b)(22):    Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:   (a) On or about November 20, 2014 at worksite 10526 Grandview Rd, Kansas City, Missouri.-employees were exposed to serious fall hazards, when  observed carrying  materials while climbing a extension ladder to access the roof work area.       Abatement certification for this violation is not required.
