29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On or about November 25, 2014, and at times prior thereto, an employee was observed working from a wooden pallet supported and lifted from the ground by a rough terrain telescopic forklift truck with unprotected side or edge and without any type of fall protection system. The working height was greater than ten feet above lower level with a roof pitch of 12:12. This condition exposed employee to fall hazards.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied. On or about November 25, 2014, and at times prior thereto, an employee was observed working from a wooden pallet platform supported by a telescopic forklift without ensuring that the entire platform was attached to the fork and forklift. The working height was greater than ten feet above lower level and employee was stepping back and forward from platform into a roof with pitch of 12:12. This condition exposed employee to fall hazards.
