29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:  On or about November 5, 2014 at the ramp where paper is taken into the offices, employees were exposed to a fall hazard of eleven feet to the lower level when the guard rail was removed and never replaced.

29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace.  On or about November 5, 2014, an employee was exposed to fall hazard on the Crown forklift  DLA 07328. The retractable lanyard used to protect the employee from falling to the lower level was damaged and not removed from service.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  On or about November 5, 2014  employees are being exposed to various hazards, to include, but not limited to being stuck in the raised position or having the forklifts stop when the employees are not following equipment procedural operations when a different driver is operating the forklift.  Employees were not familiar with all of the features of the CFAMS system.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service.  On or about November 5, 2014  the baskets on the forklifts used by the employees to pull material from the shelves were not secured in a manner that would prevent them from falling off the forks. The baskets were not attached to the forks on the forklift.

29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent.  On or about November 5, 2014 the form used to record work-related injuries and illnesses was not equivalent to the OSHA 300.   The form did not include the following information:  a.  The Job Title of the injured/ill employee (Column C);  b.  Where the event occurred (Column E);  c.  A description of the injury or illness, parts of body affected, and object/substance that directly injured or made person ill (Column F);  d.  Classification of the case as either a case resulting in death, days away from work, remained at work - job transfer or restriction, or remained at work - other recordable cases (Columns G-J);  e. Number of days the employee was either away from work or on job transfer or restriction (Columns K & L);  f.  Type of injury/illness (Columns M1-M6).
