29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Employees were installing windows on a residential structure while in a JLG Telescopic Aerial Work Platform Model 460SJ with the lanyard not attached to the boom or basket. Employees were exposed to a potential fall height greater than 25' above the ground below.
