29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.    a) General Motors LLC; On or about January 8, 2014, the employer failed to ensure that horizontal surfaces along operation 125 (cooling tunnels) in modules 1, 2, 3, and 4 were maintained in a clean and, so far as possible, a dry condition.  Employees were exposed to contact with noxious substances in that a black/brown substance composed of chemicals, such as but not limited to, Benzenamine, Phenol, O-Cresol, P-Cresol, Methyl Glutarate, and 2,4 Dimethyl Phenol leak out of operation 125 in modules 1, 2, 3, and 4 onto the doors of the cooling tunnels, electrical equipment, and onto the cooling tunnel elevator doors. Exposure routes include inhalation, skin absorption, ingestion, skin and/or eye contact.    Instance B removed per ISA and AVD modified to:    AVD for citation 1, item 1a, instance a, amended to read:  General   Motors LLC; On or about January 8, 2015, the employer did not ensure that horizontal surfaces and floors along operation 125 (cooling tunnels) in modules 1, 2, 3 and 4 were maintained in a clean and, so far as possible, a dry condition.  Employees were exposed to contact with noxious substances in that a black/brown substance composed of chemicals, such as but not limited to Benzenamine, Phenol, O-Cresol, P-Cresol, Methyl Glutarate, and 2,4 Dimethyl Phenol leak out of operations 125 in modules 1, 2, 3 and 4  onto the doors of the cooling tunnels, electrical equipment, and onto the cooling tunnel elevator doors, with the main exposure route to employees being skin absorption and skin contact      b) General Motors LLC; On or about January 8, 2014, the employer failed to ensure that Fanuc iA M-900 robots located in operations 160 (cutting) of modules 1, 2, 3, and 4 were maintained in a clean condition. Employees were exposed to flash fire hazards in that layers of aluminum dust have accumulated on surfaces of the non-intrinsically safe robots.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used:    a) General Motors LLC; On or about January 8, 2014, the employer failed to ensure that employees using tight-fitting filtering face-piece respirators were fit tested whenever a different respirator face piece was used. Employees were required to wear tight-fitting filtering face-piece respirators when entering operation 130 and 160, shake out and cutting, of modules 1, 2, 3, and 4. Employees were exposed to hazards associated with exposures to crystalline silica dust such as but not limited to silicosis.
