29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Formtight, Inc. at 11600 E 51st Ave., Denver, CO 80239:  On or before November 26th, 2014, the employer did not ensure that an energy control program, specific to the establishment, which included energy control procedures, employee training and periodic inspections was established.  This condition exposed employees conducting servicing or maintenance operations to hazards associated with unexpected energizing, startup or release of stored energy.

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them.  a) Formtight, Inc. at 11600 E 51st Ave., Denver, CO 80239:  On or before November 26th, 2014, the employer did not ensure that specific steps were developed for the placement, removal and transfer of lockout devices, with designated responsibility for them, in that alternate keys for lockout devices were readily available for any employee to use.  This condition exposed employees to hazards associated with uncontrolled hazardous energy.
