29 CFR  1910.147(f)(1): In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions was not followed: (i) Clear the machine or equipment of tools and materials in accordance with paragraph (e)(1) of this section;  (ii) Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section;  (iii) Remove the lockout or tagout devices as specified in paragraph (e)(3) of this section;  (iv) Energize and proceed with testing or positioning;  (v) Deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue the servicing and/or maintenance:   (a) FormTight, Inc. at 11600 E 51st Ave in Denver, CO: On or about October 19, 2021, when servicing and maintenance of the inline thermoformers required the machines to be energized to properly position the mold and die components, the employer did not require that employees follow steps to deenergize all systems and reapply energy control measures after the components were properly positioned. This condition exposed employees to unexpected equipment startup, which could lead to crushing and amputation of fingers, hands, and arms.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)	FormTight, Inc. at 11600 E 51st Ave in Denver, CO: On or about October 19, 2021, the downward motion of the stacker on inline thermoformer #6 created a pinch point, which was not guarded. Employees working at the stacker on inline thermoformer #6 were required to regularly place their hands near or underneath the stacker to move plastic parts. This condition could result in crushing and amputation of fingers and hands.  (b)	FormTight, Inc. at 11600 E 51st Ave in Denver, CO: On or about October 19, 2021, the flywheel and gears on the Bliss Punch Press in the thick gauge production area were not sufficiently guarded. Employees operating the Bliss Punch Press were exposed to inadvertent contact with the in-running nip points created by the belt and pulley on the flywheel and the gears, which could result in an amputation.
