29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.     WORKSHOP:  The hazardous energy control lockout/tagout procedures were generic and not written step-by-step in machine specifics for the employees during servicing and maintenance of equipment, such as ( but limited to) the CNC milling machines, lathes, and bending machines.

WORKSHOP:  The employees, as "authorized, affected and/or other" types were  not provided with training and information in the controlling of potentially hazardous and unexpected releases of energy sources (lo/to) during servicing and maintenance of equipment, such as (but not limited to) the CNC milling machines, lathes, and bending machines.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    WORKSHOP:  The ingoing nip point, created by the lower pulley and the belt of the Stephen Badger Co., deburring machine was unguarded.      ABATED DURING THE INSPECTION.

The horizontal shafts of the equipment in the following locations were unguarded:      A.     Tool Room: The horizontal shaft of the Niigata mill was unguarded.      ABATED DURING THE INSPECTION.       B.      Brazing Department:      1.      The horizontal shafts of the Bridgeport Safety Emery Wheel were unguarded.      2.      The horizontal shafts of the New Britain Machine were unguarded.      ABOVE ITEMS WERE ABATED DURING THE INSPECTION.

Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves:  The horizontal shaft ends and nuts of the equipment in the following locations were unguarded:    A.     Tool Room:  Both end nuts of the Baldor grinder/buffer were unguarded.      ABATED DURING THE INSPECTION.     B.      Brazing Department:    1.      The right end nut of the Bridgeport Safety Emery Wheel was unguarded.    2.      The horizontal shaft ends/nuts of the New Britain Machine were unguarded.    3.      Polishing wheel, unknown identification; end nuts unguarded.    ABOVE ITEMS WERE ABATED DURING THE INSPECTION.
