29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.  a) On November 24, 2014 and at times prior thereto; rear of building; employees were using electrical equipment such as but not limited to grinders which were not plugged into a ground fault circuit interrupter to protect them from electrical shock.

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights was greater than one  inch.  a) On November 24, 2014 and at times prior thereto; rear of building, employees were observed on the working platform of a scaffold system that was not fully planked, missing two planks; the height from the work platform to ground level was 18 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) November 24, 2014 and at times prior; northeast of building, employees were exposed to a fall of approximately 18 feet when accessing the scaffold system by using the cross braces.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) On November 24, 2014 and at times prior thereto; rear area of the building; employees were exposed to a potential fall of 18 ft. while working on an unguarded scaffold system.
