Oklahoma City, OK 73134. On or about November 25, 2014 employees were observed performing roofing activities without the use of fall protection. The employees were exposed to a fall hazard of approximately 30 feet.

On or about November 25, 2014 employees were observed performing roofing activities without the use of fall protection and had not been provided with a fall protection training program. The employees were exposed to a fall hazard of approximately 30 feet.
