29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective: On or about January 27, 2015 at the Executive Coach Builders Facility, employees were exposed to Electrical Shock injuries, in that the cord on a 30 inch industrial fan in use did not have a ground pin to ensure the path to ground was permanent, continuous and effective. Abatement certification is not required for this item.
