29 CFR 1910.23(c)(2): Runways four feet or more above floor or ground level were not guarded by standard railings (or the equivalent) and toeboards:   a)    Runways on both sides of a mobile-home in the production building: On or about November 25, 2014, and at times prior thereto, the employer exposed workers to fall hazards of 7 ft. 6 in., in that runways lacked handrails as employees traversed catwalks during siding, window-installation, and painting activities.

29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms:   a)    Fixed-industrial stairway near the mill-area in the mobile-home production building: On or about November 25, 2014, and at times prior thereto, the employer exposed workers to fall hazards of up to 7 ft., in that a standard mid-rail was not provided on one open side, and no handrail on the other open side as employees used the stairway to access the catwalk.     b)    Fixed-industrial stairway on the door-staging side of the catwalk in the mobile-home production building: On or about November 25, 2014, and at times prior thereto, the employer exposed workers to fall hazards of up to 7 ft., in that standard mid-rails were not provided on open sides as employees used the stairway to access the catwalk.     c)    Fixed-industrial stairway near the roof-jig platform in the mobile-home production building: On or about November 25, 2014, and at times prior thereto, the employer exposed workers to fall hazards of up to 7 ft., in that standard mid-rails were not provided on open sides as employees used the stairway to access the catwalk.     d)    Fixed-industrial stairway near the bench grinder in the mobile-home production building: On or about November 25, 2014, and at times prior thereto, the employer exposed workers to fall hazards of up to 7 ft., in that a standard mid-rail was not provided on open sides as employees used the stairway to access the catwalk.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   a)    At the mobile-home production facility: On or about November 25, 2014, and at times prior thereto, the employer exposed workers to chemical hazards for at least argon, a vinyl spackling compound, and oil-based paints and silicone, in that a written hazard communication program was not developed and maintained to at least describe how the requirements of labels, safety data sheets and employee training will be complied with.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   a)    At the mobile-home production facility: On or about November 25, 2014, and at times prior thereto, the employer exposed workers to related chemical hazards for at least oil-based paints and silicone, in that employees were not trained on topics not limited to the categories of hazards, chemical-specific hazards, appropriate personal protective equipment, labels, and safety data sheets.
