29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk shall be guarded by either:    29 CFR 1910.23(a)(8)(i): A standard railing with standard toeboard on all exposed sides, or    29 CFR 1910.23(a)(8)(ii): A floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing.      Ball Corporation, 2690 Charter Street, Columbus Ohio workplace:       a.  On or about December 1, 2014, the Seamer elevator floor hole was not protected by standard railing or a floor hole cover,  b.  On or about December 1, 2014, the Feeder elevator floor hole was not protected by standard railing or a floor hole cover, and  c.  On or about December 2, 2014, the Necker elevator floor hole was not protected by standard railing or a floor hole cover.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder:  a.  Ball Corporation, 2690 Charter Street, Columbus Ohio workplace: On or about December 2, 2014, the employer had not protected with standard railing (or equivalent protection) open sided platforms on the mezzanine above the Weld department Line 1 (around the corner from the Necker elevator.)  Due to this condition, employees are exposed to a fall hazard of approximately ten feet.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):     a.  Ball Corporation, 4960 Charter Street, Columbus Ohio workplace: On or about December 1, 2014, the employer did not require energy isolating device use to in a manner to isolate machine energy sources in the Weld department. When performing required daily duties on five production line Welders covered by this part, such as clearing jams, making adjustments and making checks, energy source isolation is inadequately performed by use of an emergency stop with a tag placed around the emergency stop fixture. Due to this condition, employees are exposed to caught-in hazards from potentially moving parts.

Ball Corporation, 2690 Charter Street, Columbus Ohio workplace: On or about December 1, 2014, the connection for the conduit attachment to the electric motor that powers the Feeder for the 80 Beader machine was pulled out/apart from the motor. Due to this condition, employees are exposed to an electrical contact hazard and increased risk for electrocution.

Ball Corporation, 2690 Charter Street, Columbus Ohio workplace: On or about December 1, 2014, in the Weld department, at the following locations machine guarding was secure and affixed properly:  a. at the Line 1 Stacker; a barrier guard was not secure and properly affixed. Allen head screws were missing and/or loose. b. at the Line 1 Slitter adjacent to the Slitter door interlock: the feeder barrier guard housing was not secure and affixed. An allen head screw was missing.
