29 CFR 1926.451(g)(4)(iii): When mid-rails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform. 301 Henry Street, Lindenhurst, NY: Employees were not protected from falling while installing new air piping from a scissor lift (Genie GS - 2032) approximately 15 ft. above the ground. No mid-rail was in use in that the entry chain at the end of the platform was not secured; on or about 12/1/14. Note: Because abatement of this violation is already documented in the case file, the employer need not to submit certification or documentation of abatement of this violation as normally required by 29 CFR 1903.19.
