The employer does not ensure all unused openings on boxes, cabinets, or fittings are effectively closed to prevent employee injury. This violation occurred on or about December 1, 2014, in the Screen Printing Press Area where employees were exposed to electrical and fire hazards while working around a pull box without a cover to close the opening. Graphic Screen Printing Productions, Inc., previously known as, Premium Graphicx, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i), which was contained in OSHA inspection number 314898206, citation number 1, item number 13 and was affirmed as a final order on November 18, 2010, with respect to a workplace located at 5512 Mitchelldale St., Houston TX 77092. Pursuant to 29 C.F.R. 1903.19, within 10 (ten) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the unused openings on electrical boxes, cabinets and fittings are effectively closed.

This violation occurred on or about December 1, 2014, at screen printing press and screen wash areas, where employees were using chemicals such as, but not limited to, Ink Wash-Up Solvent, Screen Reclamation Chemical and various Inks, without a written hazard communication program maintained at the worksite. Graphic Screenprinting Productions, Inc., previously known as, Premium Graphicx, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(e)(1), which was contained in OSHA inspection number 314898206, citation number 2, item number 4 and was affirmed as a final order on November 18, 2010, with respect to a workplace located at 5512 Mitchelldale St., Houston TX 77092. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that safety data sheets are maintained and made available to employees on all work shifts for all hazardous chemicals in the workplace.
