The employee was not secured to the boom or basket and stood on the mid-rail while fastening material; on, or about, 12/02/2014. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19. ISLAND ACOUSTICS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(iv), which was contained in OSHA inspection number 315205195 citation number 1, item number 2 and was affirmed as a final order on 03/17/2011, with respect to a workplace located at Sophia St., Bethpage, NY.
