29 CFR 1904.39(a): An oral report of an employment accident resulting in a fatality or an inpatient hospitalization of three or more employees was not made within eight (8) hours after the occurence to the nearest Area Office of the Occupational Safety and Health Administration or to the OSHA toll-free central telephone number (1-800-321-OSHA): a/ On or about 8:00 p.m., 11/29/14, one employee died following a fall from a fixed ladder. Neither the National Office, Regional Office, nor the St. Louis Area Office were notified of this fatality until approximately 9:00 a.m., 12/01/14.
