OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards as employees were operating a powered industrial truck in the facility without mounted seat belt:  a) On or about 12/2/14 at Georgia Metal Coatings Company, Norcross, GA, employees operated a propane Toyota Powered Industrial Truck,  model 7FGCU25, serial # 75474, to load and unload baskets of parts from the lines and trucks in the manufacturing area and shipping area  without seat belt mounted on the lift.    b) On or about 12/2/14 at Georgia Metal Coatings Company, Norcross, GA, employees operated the propane Toyota Powered Industrial Trucks to load and unload baskets of parts from the lines and trucks in the manufacturing area and shipping area. Employees did not use the seat belts mounted on the lifts.    Among other methods, one feasible and acceptable method to correct this hazardous condition is to install operator restraint system (seat belts) and require employees to wear them as required by ANSI/ASME B56.1 - 2009, Safety Standard for Low Lift and High Lift Trucks.

The exposure was derived from a sample taken over 457 minutes. Annual audiograms had not been conducted since 2006, on or about 12/3/14.  b) Georgia Metal Coatings Company - An employee operating line #3 was exposed to noise levels at a TWA of 96.7 dBA. The exposure was derived from a sample taken over 473 minutes. Annual audiograms had not been conducted since 2006, on or about 12/3/14.  c) Georgia Metal Coatings Company - An employee operating line the shot blaster was exposed to noise levels at a TWA of 97.6 dBA. The exposure was derived from a sample taken over 444 minutes. Annual audiograms had not been conducted since 2006, on or about 12/3/14.
