On or about 12/02/14 located at 4295 Martel Dr., Cumming, GA: Employees installing house wrap insulation on to the walls of the house- from the roof while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 25 feet and the pitch was a 6/12.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: On or about at 12/02/14 located at 4295 Martel Dr., Cumming GA: Employees were exposed to fall hazards, while installing house wrap insulation, employees were using portable ladders to gain access to the walls of the house, and the employee was standing on one foot, no hands on the ladder, way off center and misused the ladder for which it was designed.
