29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a. On or about December 02, 2014, at the above addressed job site, employees were exposed to falling 17 feet while working on the roof of a commercial building without a fall protection system in place. The employees were removing nails and preparing to lay felt material.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    a. On or about December 02, 2014, at the above addressed job site, an extension ladder was used to gain access to the roof of a commercial building that was not secured from displacement exposing the employees to a 17 foot fall.
