29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, a) The overhead platform used to add ingredients for the mixer was not equipped with a intermediate railing to prevent employees exposure to falls. b) The ingredients elevator was not equipped with a railing system to prevent material from falling off to the floor below.

29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a) On or about 30 November, 2014 employees were removing plastic bag from mixer. The power to the mixing machine AMFEC model #503, was not shut off and an employee's arm was caught in the injection door of the machine.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. a) On or about 30 November, 2014 employees were removing plastic bag from mixer. Energy isolating devices were not attached to the energy isolating device on the AMFEC mixer model #510 machine, to prevent employee's contact with running and moving parts.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: a) On or about November 30, 2014 employees who were involved in removal of jammed material inside of the AMFEC 510 mixer, they were not trained as authored employees when their duties included servicing and/or maintenance of equipment.
