29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about November 18, 2014, employees performing roofing work on a residential framed structure were exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards.   On or about November 18, 2014, the employer did not ensure employees engaged in roofing activities were trained to recognize and avoid fall hazards and the procedures to be followed in order to minimize or avoid those hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about November 18, 2014, employees performing roofing activities were using an aluminum ladder that was not extended at least three (3) feet above the landing to access the upper regions of the roof. The employees were exposed to an unprotected fall of over 20 feet to the ground below.
