29 CFR 1910.178(l)(1)(ii):     The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck:  (Construction Reference: 1926.602(d)):    a)  281 Littleton Road, Parsippany, NJ and at the Corporate Offices at 642-644 Fulton Street, Elizabeth, NJ.    The employer had not ensured the operator of the Gehl RS6-34 Telescopic Handler received training the training as required by paragraph (l) exposing the operator and other workers to injury.                                                    Violation observed on or about 12/02/2014.

29 CFR 1926.451(c)(2)(ii):  Unstable objects were used to support scaffolds or platform units:    a)  281 Littleton Road, Parsippany, NJ.    The employer had not ensured the tubular welded frame scaffold was supported in a stable fashion as concrete blocks were used in the scaffold bases on the West side of the scaffold exposing employees to injury from a scaffold collapse.                                                    Violation observed on or about 12/02/2014.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a)  281 Littleton Road, Parsippany, NJ.    The employer had not ensured employees had proper access to the scaffold's top level outrigger as the extension ladder only extended to the scaffold level below requiring employees to climb scaffold frames or crossbraces to the working level approximately 6 feet, 10 inches above. 29 CFR 1926.451(g)(1)(vi):     Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).    a)  281 Littleton Road, Parsippany, NJ.    The employer had not ensured the tubular welded frame scaffold had guardrails on open sides and ends of the scaffold exposing employees to falls of approximately 27 to 28 feet to the ground below.                                                    Violation observed on or about 12/02/2014.
