29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2): a) Bay 4 / LCS-12 / Hull 703 / USS Omaha - Section H06C; On or about December 3, 2014 and at times prior thereto the employer exposed employees to a 15 foot 6 inch fall hazard in that employees were allowed to walk and work around an unguarded open edge to a ladder access.
