The receptacle was not installed in a junction box and used as listed and labeled for use, exposing employees to the hazards associated with an electrical shock. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

The receptacle was not installed in a junction box and had exposed wire terminals, exposing employees to the hazards associated with an electrical shock. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On December 3, 2014 an employee was not protected from an electrical shock while working in the electrical closet where distribution panels not closed, exposing the employee to an electrical shock from voltages ranging between 110-480 volts. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
