On or about December 02, 2014, in the fabrication shop,  fabricators used a hand held Makita grinder with a metal cutting disk 4.5-inch diameter and a Milwaukie grinder with a metal cutting disk 12-inch diameter to cut metal for metal gates and railings. The grinders did not have a safety guard exposing employees to laceration hazards.

On or about December 02, 2014, in the fabrication shop, employees using chemicals such as, but not limited to, carbon dioxide, acetylene, argon, and aluminum welding wire and metallizing wire, had not been provided with training on the health and physical hazards associated with these chemicals.
